Rewrite User Contacts dialog

Rewrite Add Contacts dialog (User Profile->Edit Profile->Add More) to the same design as login window

Please also note the following issue:
Steps to reproduse:
1. Sign in
2. Edit the profile
3. Click the button 'Add more...'
4. Click beyond the field of dialog for adding contacts

ACTUAL RESULT: Line with buttons 'Ok' and 'Cancel' highlighted with orange (see video in the attachment)

EXPECTED RESULT: Nothing happened
